Insurance reimbursement is based on your individual insurance coverage. We are providers for several insurance plans, including Medicare and Medicaid. Please contact your individual carrier to determine coverage.

Initial Consultation
Individual 1 hour consultation Cost $125.00 - $150.00

Follow-Up Consultation
Individual 30 minute consultation Cost $95.00

Payment Policy
If insurance is filed, co-payment is due at time of service. Should payment be denied by insurance carrier, patient is responsible for all charges. For consultations not covered by insurance, full payment is due at the time of service.

We accept cash, checks, Visa and Master Card.
